Overview:
We are seeking a dedicated and compassionate Clinical Care Coordinator RN to join our team on a contract basis. In this pivotal role, you will be responsible for coordinating patient care, collaborating with healthcare providers, and ensuring that our patients receive the highest quality of services. Key Responsibilities:
- Conduct comprehensive patient assessments to develop individualized care plans that address both medical and psychosocial needs.
- Coordinate and communicate with interdisciplinary teams to facilitate seamless patient transitions across care settings.
- Monitor patient progress and outcomes, making adjustments to care plans as necessary to ensure optimal health results.
- Educate patients and their families about conditions, treatments, and post-discharge care to promote self-management and adherence to healthcare plans.
- Manage and resolve patient concerns and complaints by acting as a liaison between patients, families, and healthcare providers.
- Maintain accurate and up-to-date documentation in compliance with regulatory and organizational standards.
- Participate in quality improvement initiatives to enhance patient care processes and outcomes.
- Advocate for patients and families, ensuring they have access to necessary resources and support services.
Required Qualifications:
- Education: Bachelor of Science in Nursing (BSN) from an accredited nursing program.
- Experience: Minimum of 2 years of clinical nursing experience in a hospital or healthcare setting.
- Technical Skills: Current Registered Nurse (RN) licensure in the District of Columbia; proficiency in electronic health record (EHR) systems and Microsoft Office Suite.
- Soft Skills: Strong communication and interpersonal skills; excellent organizational and time management abilities; critical thinking and problem-solving skills; ability to work collaboratively in a team-oriented environment.
Preferred Qualifications:
- Additional Certifications: Certification in Care Coordination and Transition Management (CCCTM) or similar credentials.
- Bonus Skills: Experience with case management software; familiarity with community resources and healthcare policies.
